TL;DR: Adult ADHD affects 3-4% of the adult population, often undiagnosed. The Adult Self-Report Scale (ASRS) developed by the WHO is the most used screening tool. It evaluates inattention symptoms (forgetfulness, difficulty in organization), hyperactivity (motor agitation, talkative), and impulsivity (rapid decisions, sentence completion). A test score is not a diagnosis but indicates whether a clinical evaluation is necessary. CBT, combined or not with medication, offers effective tools for adult ADHD: organization techniques, attentional training, time management, work on associated self-esteem.

ADHD is not just a childhood disorder. In 50-70% of cases, symptoms persist in adulthood, often in modified forms. The result: many adults live with undiagnosed ADHD, attributing their difficulties to "weakness of character" or "lack of motivation."

The 3 symptom dimensions

Inattention

In adulthood, inattention is generally more visible than hyperactivity (which becomes more internal).

The ASRS test

The Adult Self-Report Scale was developed by the WHO. It exists in a short version (6 questions) and a complete version (18 questions).

For each question, 5-point scale (never to very often):

  • How often do you have trouble finishing the final details of a project, once the challenging parts are done?
  • How often do you have difficulty getting things in order when you have to do a task that requires organization?
  • How often do you have problems remembering appointments or obligations?
  • When you have a task that requires a lot of thought, how often do you avoid or delay getting started?
  • How often do you fidget or squirm with your hands or feet when you have to sit down for a long time?
  • How often do you feel overly active and compelled to do things, like you were driven by a motor?
  • A score above 14 (out of 24 for the 6 questions) suggests a probable ADHD requiring clinical evaluation.

    What is NOT ADHD

    Important to distinguish from:

    • Simple fatigue

    • Stress / burnout

    • Mild anxiety disorders

    • Depression

    • Sleep apnea

    • Thyroid disorders


    A medical and psychological evaluation is necessary to make the differential diagnosis.

    The complete clinical diagnosis

    A reliable diagnosis requires:

    • Validated questionnaires

    • Detailed clinical interview

    • Confirmation of childhood symptoms

    • Functional impact in several life domains

    • Exclusion of other causes

    Time management

    • Visual planning
    • Reminders and alarms
    • Pomodoro technique (25-minute work intervals)
    • Task subdivision

    Attentional training

    • Mindfulness meditation
    • Focus exercises
    • Distraction reduction

    Cognitive restructuring

    The associated beliefs are often:
    • "I'm fundamentally disorganized"
    • "I'm not made to succeed"
    • "I always disappoint"
    Restructuring these beliefs is essential.

    Self-esteem work

    Many adults with undiagnosed ADHD have low self-esteem after years of failure attributed to "character." The diagnosis itself often produces significant relief.

    Combined treatment

    For moderate to severe forms, medication (methylphenidate, atomoxetine) combined with CBT produces the best results.

    When to consult

    Consult an ADHD specialist if:

    • ASRS score above 14

    • Significant functional impact (work, couple, finance)

    • Childhood symptoms present (school difficulties, behavior)

    • Important suffering associated
